Case Management Cum Tracking Facility (CMTF) Project for Social Welfare Department (SWD)

This System Customization Design Specification has been developed by YGL Convergence (HK) Ltd. specifically for the Case Management Cum Tracking Facility (CMTF) Project for Social Welfare Department (SWD). The Licensing Office of Residential Care Homes for Elderly (LORCHE) of SWD is responsible for the licensing and regulation of the RCHEs and the registration of Health Workers. At present, processing of applications for license and registration of health worker is mainly based on paper files with the support of a computer system in maintaining the RCHE records, inspection history, complaint records and registered health worker records. To improve the operational efficiency and to facilitate effective monitoring of the related regulatory activities such as licensing process and routine inspection of RCHEs, a Case Management cum Tracking Facility system (CMTF or the system) is implemented to meet the requirements.

The objectives of CMTF for SWD are:

     To provide workflow and case management functions to support the operation of the LORCHE in respect of license application processing, routine inspections, complaint processing and heath worker registration;

     To monitor and track the performance of licensed residential care homes for elderly (RCHEs) in terms of risk level

     To provide management information on workload and performance, thus facilitating the identification of problem areas for improvement.
